There are significant gaps in mental health services for youth and adults in our province. We want to help fill these gaps utilizing Psychiatric Nurses.
We practice using a two-step process.
First, the nurse conducts a holistic and comprehensive
Mental Health Nursing Needs Assessment.
After we together create a picture of needs and goals, we will proceed with Care Plan Implementation. The care plan will reflect the client's unique situation and no two will be the same. We support the client and family towards their recovery goals with a variety of interventions.
When someone is admitted to hospital, nurses are the first face that greets them. It is their responsibility to perform an extensive assessment of specific client needs, and then to connect them with appropriate services, supporting them on every step of their journey.
We aim to translate this assessment into community. Utilizing a number of tools including thorough Mental Status Exams, screeners, intensive psychosocial assessment, and suicide risk assessment, the nurses at our clinic develop a tailored plan for youth and provide ongoing support. We highlight strengths in clients and families and look at all aspects of a youth's life in developing a comprehensive plan.
Your nurse will assess current medications and associated side effects. Nurses cannot prescribe medication, but will work
with primary care providers to find the right combination for you.
A fancy word for answering any questions you might have about diagnosis, medication, symptoms, or other medical jargon that is difficult to understand.
It is not the intention of our clinic to provide intensive therapy, but we are versed in the skills associated with each type. We can make recommendations for practitioners and approaches that are right for your case - but sometimes there are long waitlists. We can introduce skills to clients to prepare them for the next step.
It's critical to mental wellness to feel comfortable in the classroom to feel that you are achieving. Our team will actively collaborate with school teams, sharing information to help kids thrive at their home away from home.
Our files never close. Mental health issues can be chronic and often ebb and flow with stress or other factors. We want to make sure you can see someone you trust if you start to struggle again - before things get worse.
